What a volunteer sign-up form is for
A volunteer sign-up form collects the people who want to help, when they can help and what they would like to do, so a coordinator can match volunteers to shifts without a chain of messages.
This template asks for contact details, then two sets of checkboxes: availability by part of the week and the kinds of work on offer. A photo consent, which is optional, covers event photography. The thank-you screen welcomes each volunteer by first name and promises dates that match the availability they gave.

How to customise it
A template is a starting point: every word, question and colour stays editable.
Your roles and shifts
Edit the availability and interest options to your real shifts and roles, or use a Matrix block for days against times.
Collect what you must check
Add a Date block for date of birth, a File upload for a certificate, or a Consent block for your volunteer agreement.
Print it as a QR code
Download the form’s QR code as a PNG or SVG for posters and stalls, so people can sign up from their phone on the spot.
One sign-up per person
Allow one response per person by email address so the same volunteer is not on your list twice.
What happens after someone submits

The response arrives in Responses
Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF.

You and your team are told
The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification.

The respondent becomes a CRM contact
Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details.

They get a confirmation
Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF.

Automations and webhooks take over
Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook.
Tag volunteers by what they enjoy, for example “tree planting”, so each group gets the dates that suit it from an automation or an email to that tag.
