What a membership application form is for
A membership application form is how a club, association or society lets people apply to join, rather than sign up instantly. It collects their details, the kind of membership they want and their reasons, so the committee can review each application before saying yes.
This template is a single, short page in the Serif look. The three tiers are one-tap choices, the statement is optional so nobody is put off, and agreement to the code of conduct is required before the application can be sent. It takes no payment, because the membership team replies first; add a price when you want members to pay as they apply.

How to customise it
A template is a starting point: every word, question and colour stays editable.
Charge the membership fee
Turn on Payments with a Subscription Plan billed monthly or yearly per tier, or a one-time joining fee, so approved members pay on the same form.
Show your code of conduct
Replace the consent box with an “Agreement & e-sign” block that shows the full code, or links a PDF, with “I agree” and an optional signature.
Ask more of some tiers
Show extra questions with logic only for Supporter or Patron applicants, such as a referee or a company name.
Review applications as a team
Use the review status on each response and team notes to record the committee’s decision.
What happens after someone submits

The response arrives in Responses
Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF.

You and your team are told
The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification.

The respondent becomes a CRM contact
Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details.

They get a confirmation
Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF.

Automations and webhooks take over
Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook.
Tag each applicant by the tier they chose, so approved members can be welcomed by an automation or added to the right member list.
