# Wedding RSVP Form Template > An elegant RSVP for a wedding: name, email, a yes or no, and, only for guests who accept, how many are coming and what they would like to eat. It ends with a note for the couple. A Events & bookings template in the new Vonza Forms, “Wedding RSVP”: Elegant serif RSVP; guests and meal appear only for a yes. ## What a wedding RSVP form is for A wedding RSVP form is the online version of the reply card: each guest says whether they are coming, how many people are in their party and what they would like to eat, and the couple gets every answer in one list instead of a pile of texts. This template keeps the card short. Guests who decline answer three questions and are done; guests who accept see the guest count and the meal choice appear under their answer, because a logic rule shows those two questions only after a yes. The thank-you screen greets each guest by first name. ### Who uses it - Couples who want replies, meal choices and notes in one place before the caterer needs final numbers. - Wedding planners collecting RSVPs for several weddings, each with its own link and look. - Venues and caterers who send the RSVP on the couple’s behalf and need the meal count early. - Anyone hosting a dinner, anniversary or party who wants a reply card that looks the part. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “Ava & Sam”. ### Questions Saturday 12 September · The Old Orchard. Kindly reply by 1 August. - **Your name**: Name · required - **Email**: Email · required - **Will you join us?**: Yes / No · required · buttons “Joyfully accept” and “Regretfully decline” - **Number of guests, including you**: Number · shown by logic · 1 to 4 - **Meal choice**: Dropdown · shown by logic · Herb roasted chicken, Wild mushroom risotto, Sea bass - **A note for Ava & Sam**: Long answer ### Logic - **Guests and meal only when attending**: Shows the guest count and the meal choice when the answer to “Will you join us?” is yes. ### Button and thank-you screen - Submit button: “Send reply” - Thank-you screen: “Thank you, {{name.first}}”, Your reply means the world to us. ## How to customise it A template is a starting point: every word, question and colour stays editable. - **Your names, date and venue**: Click any text on the canvas to rewrite it: the title, the date line and the reply-by date are ordinary text, and the note label uses your names. - **Name every guest**: Swap the guest count for a Repeating group, “Questions people can answer several times”, so each guest in the party gives a name and a meal choice of their own. - **Close replies on time**: Set “Close on” in the form settings to your reply-by date. A “closes in…” notice shows in the last three days, and the form stops taking replies after that. - **Match the invitation**: The template uses the Serif look with Playfair and Lora on an ivory page. Change the accent colour, the fonts or the photo in Customize to match your stationery. ## What happens after someone submits The same Vonza Forms settings run behind every template. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Filter the Responses tab by the attending answer to see who is coming, then export to CSV or Excel for the caterer with every meal choice in its own column. A guest can fill in the form again, and you will see both replies in Responses with their times, so the latest one is clear. If you want one reply per guest, turn on one response per person by email address. ### How do I count meals for the caterer? Every reply lists the meal choice in its own column. Filter Responses to guests who accepted and export to CSV or Excel, where a spreadsheet counts each meal in seconds. ### Can I ask for each guest’s name, not just a number? Yes. Replace the guest count with a Repeating group block, add a name question and the meal choice inside it, and each guest in the party fills in their own row, up to 50 rows. ### Can I send guests a copy of their reply? Yes. Turn on the email to the respondent in the form’s delivery settings; it goes to the email the guest entered and can attach their answers as a PDF.
