# Summer Camp Registration Form Template > A two-step camp registration with payment: the camper’s name, age and week, whether they can swim, their allergies and medical notes, then the parent’s contact details, a photo consent and a choice to pay in full or in three monthly payments. A Events & bookings template in the new Vonza Forms, “Summer camp registration”: Register a child for camp; pay in full or in three instalments. ## What a summer camp registration form is for A summer camp registration form signs a child up for a camp week and collects what staff need to look after them: age, the week, swimming ability, allergies and medication, and a parent who can be reached during the day. A paid camp also needs the fee, and this one takes it on the same form. The template runs over two steps, the camper and then the parent or guardian, and ends at the payment step with two choices: pay the whole fee now, or spread it over three monthly payments. The thank-you screen uses the camper’s first name, so the parent knows exactly whose place is held. ### Who uses it - Day camps and holiday clubs taking bookings for several weeks of the summer. - Sports academies running holiday training camps for children. - Schools, churches and community centres organising a holiday programme. - Arts, music and coding camps that want parents to spread the cost. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “Pine Ridge Summer Camp”. Instalment plans need an eligible Vonza plan; pay-in-full works on any. - **Register brothers and sisters together**: Put the camper questions inside a Repeating group so a parent can add each child in one response, or add a sibling discount as a discount code. - **Cap each week**: Use “Limit responses” to close the form when the camp is full, or make one form per week, each with its own limit and link. - **Collect a signature**: Add a Signature block or an “Agreement & e-sign” block to the parent step for your terms or a medical consent, read and signed in one step. ## What happens after someone submits The same Vonza Forms settings run behind every template. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **The payment is recorded**: The buyer pays on the form before the thank-you screen. The sale appears in the Sales tab with its status, and a receipt is emailed after payment. 6. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Parents who choose three monthly payments are charged on schedule, and you can pause, resume or stop a plan from the response if a camper drops out (not for Paystack payments).
