# Photo Release Form Template > A clear model release on one screen: the person’s name and email, what you may use (photos, video, their first name, their voice), where it may appear, whether they are under 18, a parent or guardian when they are, a signature and the date. A Legal & consent template in the new Vonza Forms, “Photo & video release”: A clear model release: what may be used, where, and a signature. ## What a photo release form is for A photo release form, or model release, records a person’s permission for you to use their image. It says what you may use, such as photos, video or their voice, and where it may appear, from your website to paid advertising, and it is signed and dated. This template makes both parts checklists, so a person can agree to social media but not to paid ads, and the uses are required so a release is never blank. A logic rule asks for a parent or guardian’s name only when the person pictured is under 18. The description reminds people they can withdraw consent for future use, and the thank-you screen confirms a copy is on its way. ### Who uses it - Photographers and videographers working with models and clients. - Marketing teams shooting customers or staff for a campaign. - Schools, clubs and charities photographing events. - Podcasters and YouTubers recording guests. ## What’s in the template Every question exactly as the template builds it, page by page. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Filter Responses by the channels each person agreed to before you publish, so a photo cleared for your website is never used in an ad by mistake. Who is pictured, what may be used, where it may appear, consent for minors from a parent or guardian, a signature and a date. This template covers each one. ### Can people agree to some uses and not others? Yes. Uses and channels are checklists, so someone can agree to photos on your website but not to video in paid advertising. ### Can the signer get a copy? Yes. Turn on the email to the respondent and attach their answers as a PDF; it goes to the email they entered. ### How do I handle releases for children? The template asks whether the person pictured is under 18 and shows a parent or guardian’s name when they are. Add a second signature for the guardian if you need one.
