# Meal Order Form Template > A weekly meal-prep order in two steps: how many of each meal, priced per portion, then the delivery day, address and contact details, followed by payment with a chilled delivery fee and an optional juice pack. A Orders & payments template in the new Vonza Forms, “Meal box order”: A weekly meal-prep order with per-meal quantities, delivery fee and a juice add-on. ## What a meal order form is for A meal order form lets customers build an order from a menu, choose when it arrives and pay in one go. For meal-prep and catering businesses it replaces order messages that have to be totted up by hand. This template prices every meal per portion with a Quantity block, so the total grows as the customer adds meals, from zero to fourteen of each. The second step takes the delivery slot and a full address, plus a phone number for the driver. At payment, a chilled delivery fee is added, a cold-pressed juice pack is offered next to the pay button, and a code box takes discount codes. The thank-you screen uses the Recap look, so the customer sees what they ordered. ### Who uses it - Meal-prep companies taking weekly orders with delivery. - Caterers, home cooks and dark kitchens selling a set menu. - Restaurants taking pre-orders for collection or a set delivery day. - Farm shops and grocers selling weekly boxes. ## What’s in the template Every question exactly as the template builds it, page by page.
