# Course Evaluation Form Template > An end-of-course evaluation in calm forest green: star ratings for the course content and the instructor side by side, whether the pace was right, how likely students are to recommend the course, and comments for the instructor. A Education template in the new Vonza Forms, “Course evaluation”: End-of-course evaluation in calm forest green. ## What a course evaluation form is for A course evaluation form asks students, at the end of a course, how good the material was, how well it was taught and what should change next time. It is how instructors and course creators improve a course between one cohort and the next. This template separates the content from the teaching with two five-star ratings side by side, because a strong course can have a weak delivery and the other way round. The pace question finds courses that rush or drag, the 0 to 10 recommend scale shows whether students would send a friend, and the comments go straight to the instructor. ### Who uses it - Online course creators collecting feedback at the end of each cohort. - Training providers and coaches evaluating workshops. - Schools, colleges and tutors running end-of-term evaluations. - Companies evaluating internal training sessions. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “Course evaluation”. ### Questions - **Course content**: Rating · required · 5 stars - **Instructor**: Rating · required · 5 stars - **The pace was…**: Multiple choice · Too slow, Just right, Too fast - **How likely are you to recommend this course?**: Linear scale · 0 (Not likely) to 10 (Very likely) - **Comments for the instructor**: Long answer ### Button and thank-you screen - Submit button: “Submit evaluation” - Thank-you screen: “Thank you!”, Your evaluation shapes the next cohort. ## How to customise it A template is a starting point: every word, question and colour stays editable. - **One rating per module**: Swap the two ratings for a Matrix block, modules as rows and Poor to Great as columns, to see which part of the course needs work. - **Keep it anonymous or named**: Leave out the email question for candid answers, or add one so each evaluation joins the student’s CRM contact. - **Test what they learned**: Add a few questions with right answers and turn on Quiz mode for a pass mark and the score after submit. - **Send it at the last lesson**: Share the link in the final lesson or the closing email, or embed it on the course’s last page. ## What happens after someone submits The same Vonza Forms settings run behind every template. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **Add an email to reach them later**: This template asks for no email, so nobody is added to your CRM. Add an Email question and every respondent who leaves one becomes a contact and can get a confirmation with their answers as a PDF. 4. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Compare cohorts by exporting each round of evaluations to CSV or Excel, and read the comments for the instructor in Responses, where you can add team notes. Ratings for the content and the instructor, the pace, whether students would recommend the course and an open comment. This template asks all five on one screen. ### Can I use the evaluation for an online course? Yes. Share the form’s link in the last lesson or email, or embed it on a page; it works the same for online and in-person courses. ### Can I see which module needs work? Add a Matrix block with one row per module and a rating per column, then compare the answers in Responses or an Excel export. ### Can I turn the evaluation into a quiz? Yes. Quiz mode scores answers, sets a pass mark (70% by default) and can show the score and the right answers after submit.
