# Conference Registration Form Template > A dark, stage-lit registration for a two-day conference: who is coming, which ticket they want, which workshops they would like to join, their dietary needs and whether they will be at the evening party. A Events & bookings template in the new Vonza Forms, “Conference registration”: Dark, stage-lit registration with ticket types and workshops. ## What a conference registration form is for A conference registration form collects what an organiser needs before the doors open: who is attending, which ticket they hold, which sessions they want and anything the venue must plan for, such as dietary needs. It replaces a spreadsheet of emails with one list you can sort and export. This template asks for name, work email, company and job title, then a ticket type (in person, virtual or student) and up to two of four workshops, so popular sessions do not fill with people who ticked everything. Dietary needs and an evening party RSVP give the caterer real numbers. It has no price set; turn on Payments when tickets cost money. ### Who uses it - Conference and summit organisers who need attendee lists by ticket type and workshop. - Associations running an annual meeting with in-person and online places. - Companies hosting a customer or partner day with sessions to pick from. - Universities and training providers running a one- or two-day event with breakout rooms. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “Future of Work Summit 2026”. ### Questions 14–15 May · Lisbon & online. Tickets and the agenda arrive by email next week. ## How to customise it A template is a starting point: every word, question and colour stays editable. - **Charge for tickets**: Open Payments and add a price for each ticket, from a one-time price to a pay-what-you-want amount. Add up to five add-ons, such as a workshop pass or a dinner seat, next to the pay button. - **Cap the seats**: Turn on “Limit responses” to close the form automatically after a number of registrations, which the app suggests for seats or spots, and set “Close on” for the registration deadline. - **Change the sessions**: Edit the workshop options in place and keep the limit of two, or raise it. Rename the ticket types to your own, and drop the party question if there is no evening event. - **Split it into steps**: Insert a New page between the attendee details and the sessions, and show progress as steps or a percentage. Long registrations feel shorter on a phone this way. ## What happens after someone submits The same Vonza Forms settings run behind every template. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Work email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Use the ticket answer to tag attendees in your CRM, for example “in person” or “virtual”, so the joining details and the venue guide go to the right people from an automation.
