# Coaching Application Form Template > A bold black-and-yellow application for a personal training studio: name and email, the applicant’s goals, how ready they are to commit on a scale of 1 to 10, their experience, how often they want to train and any injuries. A Applications & intake template in the new Vonza Forms, “Coaching application”: Bold black-and-yellow application for a personal training studio. ## What a coaching application form is for A coaching application form lets a coach choose clients instead of taking whoever books first. It asks what the person wants, how committed they are and anything that limits what they can do, so the first call is about a plan rather than basic questions. This template was written for a personal training studio, but the shape fits any coach. Goals are checkboxes, commitment is a 1 to 10 linear scale from “Curious” to “All in”, and experience and sessions per week are one-tap choices. The injuries question is open, so the coach hears it in the applicant’s own words. The whole application fits on one screen. ### Who uses it - Personal trainers and strength coaches with limited client spots. - Business, life and career coaches who run an application before a sales call. - Nutrition and wellness coaches screening clients for a programme. - Online coaches selling a high-ticket group or one-to-one package. ## What’s in the template Every question exactly as the template builds it, page by page. Change, add or remove any of them in the editor. Form title: “Apply for coaching at Forge”. ### Questions - **Name**: Name · required - **Email**: Email · required - **Your goals**: Checkboxes · required · Get stronger, Lose fat, Train for an event, Move without pain - **How ready are you to commit?**: Linear scale · 1 (Curious) to 10 (All in) - **Training experience**: Multiple choice · New, Some, Lots - **Sessions per week**: Multiple choice · 1, 2, 3+ - **Any injuries we should know about?**: Long answer ### Button and thank-you screen - Submit button: “Apply now” - Thank-you screen: “Application in. Let’s go.”, A coach will call you within two days to plan your first session. ## How to customise it A template is a starting point: every word, question and colour stays editable. - **Rewrite it for your coaching**: Change the goals and the experience options to your niche, for example revenue stage for business coaching, and keep the 1 to 10 commitment scale. - **Score the applications**: Turn on Quiz mode and give points to the answers that fit your ideal client; each response then gets a score you can sort by. - **Book the discovery call**: Add a “Book a time” block that sends applicants to your Vonza booking page, or show it only to applicants who rate their commitment highly. - **Make it a conversation**: Turn on “One question at a time” with a welcome screen, so the application feels like a chat on a phone. ## What happens after someone submits The same Vonza Forms settings run behind every template. These are the ones this form uses. 1. **The response arrives in Responses**: Each response opens with every answer, the device and the time it took. Export all or some of them to CSV or Excel, or download one as a PDF. 2. **You and your team are told**: The email to you goes to you and up to five teammates on every response, with all the answers or a message you write, plus an in-app notification. 3. **The respondent becomes a CRM contact**: Everyone who leaves an email (here, “Email”) becomes a contact in your Vonza CRM, tagged with up to ten tags you choose. Existing contacts keep their details. 4. **They get a confirmation**: Turn on the email to the respondent, with your own subject and message, and attach their answers as a PDF. 5. **Automations and webhooks take over**: Use “Contact fills this form” in Automations to enrol people in a course, start an email sequence or add them to a list, or post each response to Zapier, Make or your own server with a webhook. Tag new applicants in your CRM, for example “coaching applicant”, and start a short automated email sequence while you review the application. Goals, commitment, current level, availability and anything that limits the work, such as injuries. This template asks all of these in seven questions on one screen. ### Can I only show my booking link to good-fit applicants? Yes. Add a Book a time block and a logic rule that shows it when the commitment score is high, or end the form early on a different thank-you screen for everyone else. ### Can I charge an application fee? Yes. Turn on Payments and add a one-time price; applicants pay on the form when they apply. ### Where do applications go? Into Responses, with a review status and team notes for each one. Every applicant who leaves an email also becomes a contact in your CRM.
