# How to Create an Online Form, Step by Step > Create an online form in six steps: name it, start with Vonza AI or a template, add questions and logic, set the look, publish and share the link. Canonical: https://vonza.com/blog/how-to-create-an-online-form An online form is a page that asks people questions and sends their answers to you. It can collect a name and email, take a booking, sign someone up for an event, run a survey or take a payment. You can create one in Vonza in six steps, starting from a sentence you type or from a finished template. ## Step 1: Name your form Open Forms in your dashboard and create a new form. The first screen asks for a name, such as "Summer camp registration". The name helps you find the form later and becomes the starting point for its link. Nothing is public until you publish, so you can take your time. ## Step 2: Choose how to begin You have three ways to start: - **Create with Vonza AI**: describe the form in a sentence or two. Vonza AI writes the questions, picks the right blocks and styles the form. It uses AI credits, and the wizard shows roughly how much before you start. - **Start from a template**: pick one of 61 photographic templates in 12 categories, from contact and quote forms to RSVPs, intake forms, order forms and waivers. Browse them in the [form templates](https://vonza.com/form-templates) gallery. - **Start blank**: tap the questions you want from picture tiles and build the rest in the editor. If you choose Vonza AI, the more specific your sentence, the better the form. "Client intake for a yoga studio" works; "Client intake for a yoga studio with injuries, experience level and a photo consent" works better. You see the result in a live preview and can generate again before you use it. The [AI form builder](https://vonza.com/ai-form-builder) page has more examples. ## Step 3: Add and edit your questions The editor is drag and drop, with a live preview next to it. Click any question to change its wording, make it required or add a description. Use the right block for each answer: - **Contact details**: name, email, phone and address blocks check what people type. - **Choices**: multiple choice, checkboxes, dropdowns, picture choice and yes or no. - **Ratings and scales**: stars, a 0 to 10 scale, a slider or a matrix grid. - **Files and signatures**: a file upload for documents or photos, and a block to draw a signature. Keep the form as short as the job allows. Ask only what you will use, and put the easy questions first. A long form can be split into pages; see [multi-step forms](https://vonza.com/blog/multi-step-forms) for when that helps. ## Step 4: Add logic, if you need it Logic changes the form based on the answers. A rule can show a follow-up question only when someone picks "Other", skip a page that does not apply, or end the form early on a different thank-you screen. If your form does not need any of that, skip this step. If it does, the guide to [conditional logic in forms](https://vonza.com/blog/conditional-logic-forms) has worked examples. ## Step 5: Set the look and the thank-you screen Pick one of 23 themes, set your colours and fonts, upload your logo and choose a layout: Classic, Cover (with a cover picture) or Split. Then decide what happens after someone submits: a thank-you screen, a button to another page, or a redirect. This is also the moment to set the follow-up. In the form's settings you can: - email yourself, and up to 5 people on your team, for every response - send the respondent a confirmation email with their answers as a PDF - tag everyone who leaves an email as a contact in your [CRM](https://vonza.com/crm), ready for an automation ## Step 6: Publish and share Click publish. Vonza checks the form first: for example, a payment form needs an email question and a way to pay. Once it is live, share it in any of these ways: - the link, at `/f/your-form-name` on your site - the QR code, as a PNG or SVG, for print and posters - an embed on your website, inline or as a popup, slide-in or floating button The guide to [embedding a form on your website](https://vonza.com/blog/embed-a-form-on-your-website) shows where the code goes. ## After it is live Responses arrive in the Responses tab, where you can search, filter, mark each one New, In progress or Done and export to CSV or Excel. The analytics page shows how many people opened the form, how many started, how many finished and where they left. Describe it to Vonza AI or pick a template, check the questions, and publish. Longer forms with logic or payments take longer to set up and test. ### Do I need a website to create an online form? No. Every Vonza form gets its own link and a QR code, so you can share it by email, in a message or on social media. If you do have a website, you can embed the form on it as well. ### Can I change a form after it is published? Yes. You can keep editing a Vonza form after it is live. The editor saves your changes as you go.
